Endoscopic histologisation of diminutive colorectal polyps. Are we ready for a change?

Summary

Digital chromoendoscopy used with validated classification scales allows endoscopists to predict colorectal polyp histology in vivo with a high degree of precision. The knowledge of the polyp histology without the need for biopsy offers endoscopists new strategies such as “resect and discard” and “observe and leave”. Although these strategies transfer the responsibility for correct diagnosis from the pathologist to the endoscopist, they allow us to rationalise the prevention of colorectal cancer. The authors summarise the arguments for accepting these new strategies and specify the requirements for their implementation into daily endoscopic practice. The cited international studies are complemented with data from our institution.
