Urgent surgery of thyroid gland – two case reports.

Overview
The authors describe the cases of two patients who had to undergo emergency surgery because of imminent asphyxia due to precipitate growth of the left lobe of the thyroid gland in cervicomediastinal localisation. In the first case, the cause was the transformation of Hashimoto autoimmune thyreoiditis into rapidly progressing lymphoma. In the second case, a malignant conversion of multinodular toxic goitre into an anaplastic carcinoma occurred, invading the trachea and narrowing its lumen. Both patients were immediately operated following intubation. In spite of intensive care, the first patient died 46 days after the operation. The second patient required cardiopulmonary resuscitation shortly after the operation, however it was possible to discharge her on the 6th day with a tracheostomy to a regional hospital where she died 44 days later due to cerebral stroke. The authors propose considering an early operation of nodular thyreopathy in older patients as a prevention of a malignant conversion.
