Pyogenic staphylococcal sacroiliitis diagnosed by three-phase bone scan

Overview
A case of 11-year-old girl with the acute pain of the lumbar spine, pelvis and proximal part of the left thigh is presented. Three-phase bone scintigraphy detected increased accumulation of 99mTc-hydroxydiphosphonate in the left sacroiliac joint. This result supported diagnosis of the septic inflammation of the left sacroiliac joint. The blood culture was positive for Staphylococcus aureus. Parenteral antibiotic treatment led to the decrease of inflammatory parameters and pain reduction. Due to the formation of abscess between the left hip bone and iliac muscle the pus evacuation was performed under general anesthesia. Staphylococcal sacroiliitis requires rapid diagnosis, targeted antibiotic therapy and surgical management of complications.
