Outpatient geriatrics in the Czech health care system in the eyes of geriatricians

Overview

The study analyses the causes and consequences of the current unfavourable situation in the availability of geriatric outpatient services in the Czech Republic, dealing also with the possibilities of extending their network and improving the organization of the geriatric care system. A survey was carried out among the 230 geriatricians registered in the Czech Medical Chamber. Only 48 geriatricians provide outpatient care services and, moreover, most of them only as a part of their week workload. The geriatricians’ opinion was highly critical above all concerning the readiness of the Czech Republic for the demographic aging process, the availability of outpatient geriatric care, follow-up of other services, and awareness of general public and also experts. Health insurance companies are currently not interested in contracting new providers of outpatient geriatric care. In addition to the problematic funding, persisting separation of health and social services and a low level of demand mainly due to the lack of relevant information on the possibilities and benefits of the geriatric care seem to be other major obstacles of the development of outpatient geriatrics. The future of outpatient geriatrics is closely connected with the development of community care for elderly, and must be based mainly on cooperation with general practitioners and other professionals in the field of health and social services.
