The main treatment of idiopathic thrombocytopenic purpura (ITP) in adults is based onlong-term application of corticosteroids. The different treatment modalities including surgerymay appear necessary in patients with corticoresistant ITP.Demonstrating the case of a 36 year-old man suffering from corticoresistant ITP and severethrombocytopenia the authors discuss contemporary treatment possibilities and call attentionto the preparation for splenectomy by means of activated factor VII.They used NovoSeven (NovoNordisk) after the preparation by concentrated thrombocytes had failed. Splenectomy wentproblem free, the patient healted primarily and the amounts of thrombocytes became stabilised.
