The author describes briefly the fundamental pathogenetic mechanisms of the development of autoimmune diseases as well as standard therapeutic possibilities - immunosuppression, treatment by intravenous immunoglobulin and therapeutic plasma exchange. He evaluates in more detail the advantages and disadvantages of treatment by plasmapheresis as a non-selective extracorporeal elimination method. Subsequently he describes new possibilities of semiselective elimination methods - immunoadsorption. He mentions basic types of immunoadsorbents (phenylalanine, tryprophan, staphylococcal protein A and sheep antibodies against human immunoglobulins) and mechanisms of their action (hydrophobe and affinity chromatography) along with a practical description of this therapeutic elimination method. Attention is also paid to undesirable and side-effect when using different types of adsorbents. Subsequently indications for treatment by immunoadsorption in different medical disciplines are given. Finally the author describes the position as regards the use of immunoadsorption in the Czech Republic.
