Overview

The authors describe the etiology and pathophysiology of pleural empyema. They consider the present therapeutic possibilities for this still serious disease. The choice of the therapeutic method depends on the stage of the disease, the general condition of the patient, and associated comorbidities. The basic therapeutic method includes pleural cavity drainage, lavage of the empyema cavity, intrapleural application of fibrinolytic agents, and antibiotic treatment based on cultures. In cases, where these approaches fail, surgery should be considered. The authors regard tunely initiation of treatment the most important factor, having a marked influence on the course of the disease as well as on its outcome. The more tunely the initiation of adequate treatment, the higher the probability of complete cure without complications or the necessity to use surgery.
