Overview

Thiazides have a reliable effect on reduction of blond pressure proved by long-term experience. The author describes their mechanistu of action in the nephron and assumed mechanistu of antihypertensive action. The saluretic, kaliuretic and secondary diuretic action of thiazzdds is associated with undesirable metabolic effects. The incidence and severity of undesirable effects however declined after administration of low recommended doses (12.5-25 mg/day) preserving the antihypertensive action up to a dechne of glomernlar filtration to 0.5 mUs. Thiazidy treatment calls for more frequent ambulatory and biochemickl check-up examinations. Because of their reliable effect, good tolerance, simple administration and low price they should remain a regular part of antihypertensive treatment.
