Overview
The author describes the legal framework of court ordered protective treat-ment of sexual offenders in psychiatric and sexuological out-patient settings.He presents his own definition of paraphilia and draws attention to its co-morbidity in persons who commit violent and ethical offences. He enumara-tes the shortcomings of contemporary diagnostic and therapeutic practicewith its consequences for patients, victims and society as a whole. Despiteprevious expert investigation before the court proceedings it is the duty ofthe out-patient department to check the diagnosis, supplement the documen-tation and get an unmotivated patient to collaborate in treatment. The aut-hor summarizes standard sexual diagnostic and psychodiagnosticprocedures. He differentiates methods of individual, paired and group psy-chotherapy to develop self-control and insight of the patient, regard for a dif-ferent sexual motivation. He indicates surgical and pharmacologicaltreatment with regard to the danger of the assessed abnormality. He remindsof essential social support and social control of patients having protectivetreatment and the importance of long-term follow-up.
