Post-COVID-19 Patient Care from the Perspective of Experts Across Fields

For many patients with COVID-19, overcoming the acute phase is crucial, but we now know that the disease may not end there. Some patients even manage the acute stage with very mild symptoms, and significant difficulties arise later in the form of 'post-COVID syndrome.' This issue was discussed in detail at a January webinar by leading experts from various fields.

Follow-up care for patients who have had COVID-19 is challenging and has many specifics in different patient groups. One of the webinars on this topic took place at the Petrof Gallery in Hradec Králové on January 26, 2021 (the full recording of the event is available here). The discussion featured prof. MUDr. František Kopřiva, Ph.D., from the Children's Clinic of the Faculty of Medicine at Palacký University and University Hospital Olomouc, doc. MUDr. Vladimír Koblížek, Ph.D., from the Pulmonary Clinic at the Faculty of Medicine, Charles University and University Hospital Hradec Králové, epidemiologist and vaccinologist prof. MUDr. Roman Prymula, CSc., Ph.D., and general practitioner MUDr. Norbert Král.

Comprehensive View on Pathophysiology and Immune Response

Professor Kopřiva spoke about the development and current understanding of coronavirus infection and the immune response to it. One of the important points he focused on was the cytokine storm. He characterized the dynamics of the response during the disease and its significance for the clinical picture and outcome. He showed the connection of the infection with many pathophysiological processes and described typical findings in various examinations and their biological basis. He also focused on some specific patient groups and the reasons why they are more susceptible to the infection, as well as the situations that can influence the interaction of the human body with the virus. His comprehensive approach to the topic also included an overview of preventive and therapeutic approaches, the justification, and potential benefits of which he illustrated.

Post-COVID Syndrome

Assistant Professor Koblížek's lecture was based primarily on the real experience of a pulmonologist with post-COVID syndrome. He provided a detailed description of the current understanding of this problem, identified the proportion of patients who may encounter it, and also indicated which medical fields would be involved due to the polymorphism of symptoms. He compared post-COVID patient care in the Czech Republic with care in other countries, such as India and the United Kingdom. Particularly, the comprehensive approach to the problem, taking into account all the needs of post-COVID patients, which the United Kingdom has adopted, was very inspiring and encouraging in his lecture. He also demonstrated the algorithms of his workplace, justified the choice of diagnostic and treatment procedures, and concluded his speech with a sad but truthful statement about the prognosis of post-COVID syndrome.

Practical Aspects of Vaccination

Professor Prymula focused on the issue of vaccination. He described the practical aspects associated with the use of different vaccines, the process of their approval, and the challenges of their use. He presented the national vaccination plan, which is divided into phases according to goals and the groups to be vaccinated in terms of impact on mortality reduction, vaccination of critical infrastructure, and subsequently other population groups.

From the Perspective of a General Practitioner and a Patient

Dr. Král selected several aspects of COVID-19 for his presentation, from the viewpoint of a general practitioner (both as a professional and as a person), but also from the perspective of a patient with post-COVID syndrome, who sometimes has difficulty finding help and faces accusations of malingering. He also introduced a project focused on the early detection of chronic obstructive pulmonary disease (COPD).

At the end of the webinar, the speakers discussed the presented topics and answered questions from the online audience.
