Floating clavicle - case report

Overview
Goal:
Unilateral sternoclavicular and acromioclavicular dislocation - floating clavicle - is a rare injury, its treatment is controversial. Goal of this study is to establish an optimal algorithm of treatment of this injury.
Methods:
The authors present their own case of 41 year old woman with this diagnosis and they made a review of current literature related to this topic.
Result:
In this case was proceded as follows: It was made an open reduction of acromioclavicular joint and its osteosynthesis with tension band, sternoclavicular joint was left dislocated anteriorly. Six months after this injury the patient has normal range of shoulder motion, but has occasional pain and sense of instability in the sternoclavicular joint during heavy load, acromioclavicular joint is pain free, she is satisfied with the result.
Conclusion:
Floating clavicle is a rare injury, an optimal universal algorithm of treatment is currently nonexistent, it should be induvidualised. For elderly patients with low functional demand, conservative treatment is probably suffitient, for young active individuals we recomned rather operative treatment after considering possible risk of this procedure.
