COMPARATIVE STUDY OF LASER ASSISTED LATERAL RELEASE OF PATELLA AND CONVENTIONALLY PERFORMED RELEASE

Overview

Purpose of the study:
The authors performed a comparative study on a group of patients who underwent lateral release of the patella of both knees. One knee was operated conventionally with mechanical tools and the other with laser assisted arthroscopy. The study compares the results after surgeries and evaluates which method provides the better outcome.

Material:
A group of 22 patients with femoropatellar disorders of both knees was investigated. The average age of patients was 42 years. All had no associated intraarticular diagnosis.

Methods:
All patients were operated under general anesthesia, using standard arthroscopic instruments, shaver, and arthroscopic fluid pump. Laser assisted release: intraarticular performed release using laser. Conventionally performed release: extraarticular performed release using mechanical tools. All laser assisted surgeries were performed with 2.1 um Holmium: YAG laser Sharplan 2040 with power output 2.5W – 40W. No multiple intraarticular procedure was performed at the same time.

Results:
In the laser – treated group, the swelling was 30 % lower, pain 40 % lower, the use of crutches shortened by 5 days and the recovery time shortened by 7 days. Aspiration of intraarticular fluid was unnecessary and all patients preferred laser assisted release.

Discussion:
Laser assisted release is more soft tissue conserving than mechanical tooles and photothermal effects can effectively decrease postoperative bleeding. These factors contribute to lower swelling and pain, and shorten recovery time.

Conclusion:
arthroscopic lateral release of patella with assistance of 2.1 um Holmium: YAG laser provides more benefits than conventionally performed release.
