Introduction:
Eardrum reconstruction is indicated in various disease states concerning middle ear as well as in isolated eardrum cases. The objective of the reconstruction is to restore the natural barrier between the external and middle ear and to improve hearing.
Material and methods:
In the course of seven years the authors performed myringoplasty for diseases of eardrum in 276 patients. The paper evaluated functional results of myringoplasty by the underlay technique with the use of fascia from temporal muscle in three groups: otitis media chronica mesotympanalis – the stabilized form, granular myringitis and the blunting of tympanic membrane.
Results:
Hearing loss with comparable bone-air gap was detected before the surgery in all three groups. There was the most significant improvement of hearing in the otitis media chronica myringitis stabilized form, where the bone – air gap is 5 – 11 dB. Less favorable results were reached in the granular myringitis group, where otitis media chronica mesotympanalis group the mean bone-air gap is 17-23 dB. The least favorable results were in the blunting group, where the mean bone-air gap is 18-28 dB.
Conclusion:
The best conditions for healing and subsequently the functional postoperative results are in the otitis media chronica mesotympanalis group –the stabilized form. In the granular myringitis group and the blunting group the eardrum membrane there are significantly worse postoperative results, as also indicated by the influence of the eardrum itself on healing and postoperative functional results.
