Overview
Clinical pharmacist is a specialist in the therapeutic use of medications. His main concern is optimization of medication therapy. He participates in ward rounds, systematically evaluates the medication therapy and provides pharmacoterapy recommendations to physicians. Clinical pharmacists contribute to safety of medication therapy in a healthcare facility. In the Centre of Gerontology in Prague, she is a member of a multidisciplinar health care team.
