We present a case report of a 14 year old boy with onset of urinary tract infections and secondary enuresis. Uroflowmetry revealed intermittent flow with a flat curve. There was significant post-void residual volume on ultrasound. Based on these findings we suspected subvesical obstruction and recommended a cystourethroscopy under general anesthesia. A posterior urethral valve was found and treated with cold knife incision. The diagnosis of posterior urethral valves can be difficult because of the wide variety of presenting symptoms and different age of clinical manifestation in pediatric patients. Even with lower urinary tract symptoms improvement after surgery, long term clinical follow-up is essential, as subvesical obstruction might persist and progress to chronic renal failure.
