Penetrating brain injury caused by nail of pneumatic gun

Overview
In this casereport the authors describe a 29yearold man of Ukrainian origin. His brain was injured with a nail of pneumatic gun. Patient was conscious after injury, Glasgow Coma Scale score of 15, without neurological deficit. The only symptom was headache. CT scan revealed the presence of nail intracranialy, trajectory of which went through nasal cavity, anterior skull base, frontal lobes and the tip reached anterior part of Willis circle in the level of anterior communicating artery. Nail removal has been performed under control of both anterior cerebral arteries with use of pterional craniotomy. Dural defect was covered by dural substitute and sealed using fibrin glue. Primary healing was achieved and the patient was discharged home without neurological deficit.
