A dislocation of voice prosthesis to prevertebral space – a rare complication

Overview

We report a case of an unusual postoperative complication in a laryngectomized patient, who had undergone a secondary voice prosthesis insertion. A pharyngocutaneus fistula as a complication after laryngectomy had developed followed by an oesophageal stenosis which required repeated dilatations. At a regular follow-up 6 weeks after prosthesis insertion, it was not visible in a tracheoesophageal fistula and was displayed within a prevertebral space by paraclinical examination. The patient underwent prosthesis extraction and evacuation of a minor abscess above the prosthesis under general anesthesia. Another prosthesis insertion was not indicated and the patient uses buccal speech for communication. The most probable causes of post insertion complication were previous radiation and the oesophageal stenosis. The secondary voice prosthesis insertion should be indicated with careful consideration in patients with previously complicated healing (pharyngo – cutaneous fistula) or (chemo)radiation.
