Atypical form of Meniere’s Disease

Overview

The cochlea hydrops is an unusual variant of Meniere’s disease which may become manifest in hearing disorder, tinnitus and a feeling of fullness or overlay. The dizzeness attacks are missing in these patients.

The paper presents a group of 16 patients having been treated at our workplace for cochlea hydrops over the last two years.

All these patients underwent a conservative treatment. The medicament therapy proved to be highly efficient in four where hearing adjusted to normal condition. In three other patients the treatment was supplemented with intratympanic administration of depot corticoids. The conservative therapy was not effective in five patients and four of them therefore underwent a surgical therapy – decompression of saccus endolymphaticus.

Basic principles of Meniere’s disease treatment are based on the development stage. Medicament therapy is usually successful in the first and second stage, whereas surgical treatment is appropriate for the third and fourth stage of the disease. In case of conservative therapy failure, the surgical treatment may become useful even in the second stage of the disease. The therapy and approach to the patients should be individual in spite of accepted therapeutic standards.
